The students who are unable to appear for the semester examinations due to medical reasons should fill the attached form and hand it over to the Chief Medical Officer (CMO) / University Medical Officer (UMO) of the University of Colombo together with the valid medical certificate or by post.
The medical form and the medical certificate must be certified by the Chief Medical Officer, so please refrain from sending the medical certificate to other parties. The students should make sure that all medical certificates should reach the Medical Center of the University of Colombo within fourteen (14) working days from the examination date.
